Comparison of high flow nasal cannula oxygen therapy with noninvasive ventilation

Abstract
Although the invasive mechanical ventilation is inevitable in patients with respiratory distress, noninvasive oxygen therapy should be considered before applying tracheal intubation and mechanical ventilation due to various complications of invasive ventilation. Recently, noninvasive ventilation (NIV) and high flow nasal cannula (HFNC) oxygen therapy are more increasingly used if they are available than conventional oxygen therapy in clinic. As both NIV and HFNC oxygen therapy can be applied in similar clinical situations (acute respiratory distress, pulmonary edema, post operation respiratory failure and postextubation), the comparative studies of two methods have been performed. Thus, in this review, we examed a series of randomized studies about the effectiveness, adverse effects and tolerance of two methods. In conclusion, HFNC oxygen therapy is not inferior to NIV in effectiveness and is more favorable in tolerance and adverse effects than NIV. Moreover, HFNC could be applied to all types of respiratory distress, but NIV has many contraindications. Finally, we suggest that HFNC can substitute NIV, generally but in some cases who cannot maintain with HFNC only, NIV will be considered as the second option, selectively. In South Korea, the medical insurance has supported the cost of home ventilator, recently and the devices are still developing. So in cases who need long term respiratory support, NIV might be a better option.
